Submitting Medical Claims
While medical providers handle most in-network and out-of-network claims, some out-of-network providers may require you to submit the claim.
If you need to submit a medical claim, fill out the Benefit Trust Fund Member Claim Form (PDF) and send the completed form to CIGNA using the mailing address shown on the back of your medical ID card.
You must submit a separate claim form for each provider and for each dependent. Complete the claim form in full, including your member ID number and dependent information, where applicable, and submit your signed form along with an itemized bill from the medical provider. Instructions for claims submission are on the Member Claim Form. You must submit a claim within one year after the medical expense is incurred for the claim to be processed.

Request a Medical and Prescription Drug Cards
Medical ID Card
Participants in a National IAM Benefit Trust Fund medical plan will receive a CIGNA medical ID card in the mail (a separate ID card is provided for the covered employee and each eligible enrolled dependent). Your medical ID card has your member number and important contact information. Present your medical ID card whenever visiting a medical provider.
If you need health care before you receive your ID card, you can log into CIGNA's website and print a temporary ID card. You can also access CIGNA’s website to request a replacement ID card or an additional ID card for any covered participant.
Prescription ID Drug Card
Present the prescription ID drug Card along with your prescription(s) ate a participating pharmacy. The program offers an average savings of 20 percent off the regular retail price. Finding a participating pharmacy is easy – nine out of 10 pharmacies nationwide participant in the program, including all major chains and many independent pharmacies.
